Passport and Citizenship Services
All passport services must be applied for each person at the US Embassy.
Adult Renewal
If your most recent passport: was issued after your 16th birthday; was issued less than 15 years ago; is currently in your possession; has the same name you currently use
Requirements
- Applicant’s most recent passport
- Fee of $75
- Completed form DS-82

Adult Renewal
If your most recent passport: is lost or stolen OR was issued before your 16th birthday
Requirements
- Applicant must be present at the Embassy
- Applicant’s most recent passport (if it’s in your possession)
- Birth certificate/CRBA or other proof of identity (if your passport is not in your possession)
- Completed form DS-11
Completed form DS-64 (if your most recent passport was lost or stolen)__________________________________________________
Minor Passport Issuance (for children under the age of 16)
Requirements
- Both parents and the child must be present at the Embassy (if one parent absolutely cannot be present, form DS-3053must be submitted)
- Child’s birth certificate/CRBA
- Both parents’ passports
- Copy of the parents’ marriage certificate (and termination of prior marriages certificate, if applicable)
- Photos of the child growing up since last passport issuance (if the application is a passport renewal)
- Completed form DS-11
- All passport applications require 2 photos (2” x 2”/ 5cm x 5cm) in color with a white background.

The fees are payable in Ariary or US Dollars (cash and credit card are accepted).
The Consular Section is open for non-emergency ACS services weekday mornings from 8am – 11:30am. Services in the afternoons are by appointment only. Please contact us via e-mail to make an appointment.
